Make the record useful.

Eleven reusable forms with fictional worked examples. Download blank versions for practice; downloads do not submit evidence, open an assessment or create a learner record.

Use and privacy

Keep completed learner evidence private. Use exercise identifiers and only the information needed for the task. Public examples are formative; active summative variants require restricted assessor handling. A form is not proof of completion without relevant individual observation.

Crew brief

Fictional worked example

Move the six training crates from Assembly to Receiving. Aster coordinates; Relay is backup. Each changed instruction needs read-back. STOP pauses out of character. If contact is lost, hold at the agreed rendezvous and use the backup channel. Receiving access remains to be confirmed.

Blank form fields

  • Objective and role: __________________
  • Location and destination: __________________
  • Decision owner and backup: __________________
  • Communications and check-back: __________________
  • Stop signal and recovery: __________________
  • Outstanding work: __________________

Movement order

Fictional worked example

Aster carries four crates; Relay carries two. Each vessel has its own custodian. Both acknowledge the alternate before departure. Lost contact means hold, not chase. The exercise lead may withdraw; any participant may stop. Receiving confirms capacity for six.

Blank form fields

  • Vessels / crew contacts: __________________
  • Route / alternate: __________________
  • Consignments / custodians: __________________
  • Rendezvous / lost-contact action: __________________
  • Withdrawal authority: __________________
  • Receiving acknowledgement: __________________

Dispatch and receipt manifest

Fictional worked example

Training crates: six dispatched, five received, one inaccessible. Relay retains follow-up responsibility and will verify access at the agreed next check. The receiving total remains five; no quantity is invented to balance the sheet.

Blank form fields

  • Item / exercise identifier: __________________
  • Dispatched quantity: __________________
  • Custodian: __________________
  • Received quantity: __________________
  • Inaccessible / missing quantity: __________________
  • Follow-up owner and next check: __________________

In-game casualty handover

Fictional worked example

Volunteer C-02 located at the training pickup. The verified game interaction was attempted but its effect is unconfirmed. Transport accepts the handover with that limitation. This fictional example is not real medical instruction and records no real health data.

Blank form fields

  • Consenting exercise identifier: __________________
  • Last confirmed location / game status: __________________
  • Validated equipment / build: __________________
  • Actions actually performed: __________________
  • Observed effect / uncertainty: __________________
  • Transport / receiving acknowledgement: __________________

Supply request

Fictional worked example

Request twelve training containers; available capacity is eight. The requester accepts six priority and two support containers. Four remain deferred with the requester as decision owner. No substitute is loaded without acknowledgement.

Blank form fields

  • Requesting role / purpose: __________________
  • Item / unit / quantity: __________________
  • Priority rule: __________________
  • Receiving point / latest useful arrival: __________________
  • Capacity / substitution approval: __________________
  • Unmet request owner: __________________

Decision brief

Fictional worked example

Choose staged movement or wait for another vessel. Only one vessel is confirmed. Recommend staged movement unless the second is confirmed before the briefed hold point. The source allows the exercise but gives no authority to bind outside organizations.

Blank form fields

  • Decision / objective: __________________
  • Authority and limits: __________________
  • Option A / resources: __________________
  • Option B / resources: __________________
  • Assumptions / reversal threshold: __________________
  • Recommendation / unresolved question: __________________

Incident account

Fictional worked example

Observed: Relay did not acknowledge the changed destination. Reported: another crew heard a reconnect notice. Uncertain: whether Relay received the change. Response: hold and use the backup contact. Do not describe the missing acknowledgement as deliberate refusal without evidence.

Blank form fields

  • Exercise reference / event time: __________________
  • Direct observations: __________________
  • Reported claims / sources: __________________
  • Assumptions and uncertainties: __________________
  • Immediate response: __________________
  • Correction / review route: __________________

Assessor observation sheet

Fictional worked example

FO 214 arrival criterion: candidate recorded five received and one inaccessible, assigned follow-up and obtained receiving acknowledgement. Demonstrated for reconciliation. A separate lost-contact outcome was not observed after server interruption: interrupted, not failed.

Blank form fields

  • Course / curriculum version: __________________
  • Candidate exercise identifier / role variant: __________________
  • Outcome and published criterion: __________________
  • Individual action / evidence reference: __________________
  • Finding / reason: __________________
  • Practice task / assessor / independent review contact: __________________

Review request

Fictional worked example

Request review of an interrupted FD 110 support observation recorded as a failure. The exercise log shows the server disconnected before recovery could be observed. Ask for reasons against the published criterion. An independent reviewer, not the original assessor, determines the appeal.

Blank form fields

  • Course / version / decision reference: __________________
  • Contested criterion: __________________
  • Reason for review: __________________
  • Evidence reference (private): __________________
  • Requested clarification: __________________
  • Conflict-free reviewer / acknowledgement: __________________

After-action improvement log

Fictional worked example

Objective: all candidates demonstrate handover. Two were unobserved because the group exceeded evaluator capacity. Delivery-design issue: reduce the next group or add an evaluator. Programme lead owns the change; verify individual coverage in the next pilot.

Blank form fields

  • Objective: __________________
  • What happened / evidence: __________________
  • Cause category: __________________
  • Corrective action: __________________
  • Responsible role: __________________
  • How the correction will be verified: __________________

Practical readiness check

Fictional worked example

No build is supplied by this example. Before opening EN 210, reproduce the exact fault, intervention and effect indication on the actual equipment. If any step is unavailable, record not ready; a roadmap reference is not test evidence.

Blank form fields

  • Course / version / outcome: __________________
  • Actual LIVE build and test date: __________________
  • Vessel / configuration / tools / location: __________________
  • Reproduced action and observed result: __________________
  • Limitations / tested by: __________________
  • Patch trigger / retest decision: __________________

Custody
Acknowledged responsibility for an item or task.
Inject
A facilitator-controlled scenario change linked to an objective.
Interrupted
Conditions prevented valid observation; not a learner failure.
Endorsement
A scoped role/equipment demonstration, not universal authority.
